Can You Build Muscle While Taking a GLP-1?

This is one of the questions I get most from patients who care about how they look and feel, not just what the scale says. They want to know if it is even possible to build muscle while on a GLP-1, or if the medication makes it a lost cause. The answer is more encouraging than most people expect, but it comes with some important nuance.

The honest answer: it depends on what you mean by "build"

Here is the distinction that clears up most of the confusion. Preserving the muscle you already have while you lose weight is very achievable, and it should be your primary goal. Building significant new muscle while in a calorie deficit is harder, because muscle growth generally favors eating enough to support it, and weight loss means eating less.

That said, it is not all or nothing. Some people, particularly those newer to resistance training, can build modest amounts of muscle even while losing fat. But for most people on a GLP-1, the realistic and worthy goal is to hold onto the muscle you have while the fat comes off. Do that, and you will look and feel dramatically better than someone who lost the same weight without protecting their muscle.

Resistance training is non-negotiable

If you want to keep your muscle, you have to give your body a reason to keep it. That reason is resistance training. When you challenge your muscles against resistance, you signal to your body that this tissue is needed and should not be broken down for energy during your calorie deficit.

This does not require living in a gym. Two or three sessions a week that work your major muscle groups is enough for most people to see the benefit. Bodyweight exercises, resistance bands, dumbbells, or machines all work. What matters is that you are challenging your muscles with enough effort that the last few repetitions feel genuinely hard, and that you stay consistent over time.

Protein is the other half of the equation

Training gives the signal, protein provides the material. You cannot preserve or build muscle without enough protein, and on a GLP-1, falling short is easy because your appetite is suppressed. Prioritize protein at every meal and aim higher than feels natural.

As for the popular idea of precise protein timing, the anxiety around it is mostly overblown. The old belief that you must consume protein within a narrow window right after training, or the effort is wasted, does not hold up well. What matters far more is hitting enough total protein across the day. Spreading it reasonably across your meals is a sensible approach, but do not lose sleep over the exact clock. Total intake is the thing.

The common myths worth dropping

A few beliefs get in people's way, so let me address them directly.

That lifting weights will make you bulky. For the vast majority of people, especially in a calorie deficit, this simply does not happen. Resistance training helps you preserve lean muscle, which is what gives your body shape and tone as fat comes off.

That cardio is enough on its own. Cardio has real benefits, but it does not send the muscle-preserving signal that resistance training does. If muscle matters to you, resistance work is the priority.

That you are too old, or too new, to bother. Neither is true. People build and preserve strength at every age, and beginners often see the most noticeable early progress. It is rarely too late to start.
