Constipation on GLP-1 Medications: Prevention, Treatment & When to Worry

If things have slowed down since you started your GLP-1, you are in very good company. Constipation is one of the most common complaints I hear from patients on semaglutide and tirzepatide, and while it is uncomfortable, it usually makes sense once you understand what is happening. Better still, there is a lot you can do about it.

Why GLP-1s slow your digestion

Part of how these medications work is by slowing down how quickly food moves through your stomach. That slower pace is part of what keeps you feeling full longer, which is helpful for appetite. But that same slowdown continues further along, and when things move more slowly through your system, stool spends more time in the colon. The longer it sits, the more water gets pulled out of it, and the harder and drier it becomes. That is the mechanism behind most GLP-1 constipation.

On top of that, you are eating less, which means less fiber and less food volume moving through to keep things going. And because appetite is down, fluid intake often drops too. Slower digestion, less fiber, and less water tend to arrive together, and the result is predictable.

Prevention comes first

The good news is that most GLP-1 constipation responds well to a few consistent habits. These are the same things I coach my patients on before we consider anything else.

Fiber matters more now, not less. When you are eating smaller portions, it is easy to lose the fiber you used to get without thinking about it. Prioritize fiber-rich foods like vegetables, fruit, beans, and whole grains within the smaller amount you are eating. If you add fiber, add it gradually, because a sudden jump can cause bloating and gas. The goal is to aim for 24g of fiber daily.

Fluids do the quiet work here. Fiber needs water to do its job, and without enough of it, more fiber can actually make things worse. Drink more than feels necessary, especially as you increase fiber.

Movement helps things move. Even a daily walk supports healthy digestion and gut motility. You do not need a workout program, you need consistency.

Magnesium is worth knowing about. Magnesium supports normal digestion and regularity, and it is one of the gentler ways to encourage things along. It also tends to run low when overall food intake drops, so it is doing double duty for a lot of people on a GLP-1.

Constipation on a GLP-1 is common and usually explainable. Your digestion has slowed, you are eating less fiber, and you are likely drinking less water, all at the same time.
