GLP-1s and Aging: What Older Adults Should Consider

GLP-1 medications can be a valuable tool at any age, but if you are an older adult, there are some specific things worth thinking about. The medication works the same way, but an aging body has different priorities during weight loss, and getting those right makes the difference between losing weight in a way that serves your long-term health and losing weight in a way that quietly costs you. I want to walk you through what matters most.

I am Dr. Jennifer Mbianda. I am triple board-certified in family medicine, obesity medicine, and aesthetics, and I work with older patients on these medications. Here is what I want you paying attention to.

Muscle matters even more as you age

We naturally lose muscle as we get older, a process called sarcopenia, and it starts earlier than most people think. When you add rapid weight loss on top of that, the risk of losing meaningful muscle goes up, because some of what you lose during weight loss can come from muscle unless you actively protect it. For an older adult, that muscle is not just about strength. It is about staying mobile, staying independent, and avoiding the frailty that makes everything harder down the road.

This is the single most important consideration on this list. Protecting your muscle is not optional as you age, it is the whole game.

Which means protein and resistance training are non-negotiable

The two things that protect muscle become even more important with age, not less. Prioritize protein at every meal and aim higher than feels natural, since older adults often need more protein to hold onto muscle, not less, and appetite suppression on a GLP-1 makes falling short easy. Pair that with resistance training a couple of times a week. It does not need to be intense, and it is genuinely never too late to start. Beginners in their sixties, seventies, and beyond build and preserve strength, and the benefit to how you feel and function is real.

Bone health deserves attention

Bone density declines with age too, and rapid weight loss can affect bone. For older adults, and especially postmenopausal women, this is worth keeping on the radar. Adequate protein, calcium, vitamin D, and resistance training all support bone health, and it is a good thing to discuss with your doctor as part of your plan rather than an afterthought.

Do not under-eat, even though it is easy to

On a GLP-1, appetite drops, and for older adults who may already have a smaller appetite, this can tip into eating too little without noticing. Under-eating accelerates muscle loss and widens nutrient gaps, which is exactly what you are trying to avoid. Eat regular, balanced meals built around protein even when you are not hungry. Your body needs the fuel and the nutrients whether or not your appetite is asking.

Protecting your nutrition takes more intention now

When you are eating less and your body is aging, the nutrient gaps that come with reduced intake matter more. Getting enough of the nutrients that support energy, muscle, and bone becomes harder and more important at the same time. Eat nutrient-dense foods within your smaller portions, get periodic bloodwork so you and your doctor can see your actual levels, and consider daily nutrient support to help protect your intake. This is part of why I formulated the Jealthy Weight Loss Support Multivitamin, to help people eating less protect their nutrient intake, which is a job that only gets more important with age.

Keep your whole care team in the loop

Older adults are more likely to be managing other conditions and taking other medications, which means the potential for interactions and the need for coordinated care is higher. Make sure the doctor prescribing your GLP-1 knows everything else you take, and keep your follow-up appointments. This is where careful, individualized medicine really earns its value.

The bottom line

A GLP-1 can absolutely be appropriate as you age, but the priorities shift. Protecting muscle and bone moves to the center, protein and resistance training become non-negotiable, under-eating becomes a real risk to guard against, and coordinated care with your full medical picture matters more than ever. Lose weight in a way that keeps you strong, mobile, and well nourished, and you protect not just your weight but your independence and quality of life for years to come.
