What I Want Every Patient to Know Before Starting a GLP-1

When a patient decides to start a GLP-1, we have a conversation first. Not a rushed one. There are a handful of things I want every person to understand going in, because the people who start with the right expectations tend to have a smoother, safer, and more successful time of it. If you are considering one of these medications, think of this as the conversation I would want to have with you.

I am Dr. Jennifer Mbianda. I am triple board-certified in family medicine, obesity medicine, and aesthetics, and I start patients on these medications regularly. Here is what I want you to know before you begin.

The medication is a tool, not the whole plan

This is the first thing I say, because it reframes everything else. A GLP-1 quiets your appetite, which is powerful, but it does not build the habits that carry you long term. The medication and the work go together. Protein, movement, hydration, and good nutrition matter just as much on the medication as off it. Go in expecting to be an active participant, not a passenger, and you will get far more out of it.

Eat for nutrition, not just less

The medication will make you eat less. Your job is to make sure the less you eat is working hard for you. When your appetite drops, it is easy to under-eat protein and coast on whatever is easy, and that is exactly when nutrient gaps start to open up. Lead with protein at every meal, choose nutrient-dense foods within your smaller portions, and treat this as a priority from day one, not something to fix later once symptoms show up.

Some side effects are normal, and most are manageable

Nausea, constipation, and some fatigue are common, especially early and after dose increases. Knowing that in advance keeps you from panicking when it happens. Most of it eases as your body adjusts, and most of it responds to the basics: smaller meals, more fluids, fiber, and protein. Knowing what is routine also helps you recognize what is not, which is just as important.

Protect your muscle from the start

When you lose weight quickly, some of that loss can come from muscle unless you actively work against it. This matters for your strength, your metabolism, and how you feel and look. Two things protect it: enough protein, and resistance training a couple of times a week. Start these early rather than trying to rebuild later. Your future self will thank you.

Your appetite is no longer your guide

On a GLP-1, you may simply not feel hungry, and that can quietly lead to eating far too little. Do not use hunger as your only signal. Eat regular, balanced meals even when you are not especially hungry, because your body still needs fuel and nutrients whether or not your appetite is asking for them.

Screening and follow-up are not formalities

The candidacy conversation and the regular check-ins are where good outcomes actually come from. Be honest about your full history and your family history, keep your appointments, and get your bloodwork done. The patients who do best are the ones who are screened carefully and followed consistently. This is not red tape. It is the medicine working the way it should.

Have a long-term view, including the eventual transition off

Before you even start, it helps to hold the whole arc in mind. You may be on this medication for a season or for the long term, and either way, the habits you build now are what carry you. If you do eventually come off, the transition goes best when your protein, training, and structure are already solid. Starting well sets up ending well.

Before you start a GLP-1, know this: it is a tool that works best alongside your effort, not instead of it. Eat for nutrition, protect your muscle, expect some manageable side effects, do not rely on appetite as your guide, and take screening and follow-up seriously.
