Why Am I So Tired on Ozempic, Wegovy, Mounjaro, or Zepbound?

If you started a GLP-1 and suddenly feel like you cannot make it past 2pm, you are not imagining it, and you are not doing anything wrong. Fatigue is one of the most common things my patients bring up once they are a few weeks into semaglutide or tirzepatide. The good news is that most of the time, it is explainable. And once you understand what is driving it, you can usually do something about it.

I am Dr. Jennifer Mbianda. I am triple board-certified in family medicine, obesity medicine, and aesthetics, and I treat people on these medications every single day. Here is what I tell them about the tiredness.

Your calorie intake dropped fast, and your body noticed

GLP-1 medications work in large part by quieting appetite. That is the point. But when your appetite drops, your intake usually drops with it, often faster than you realize. You are simply eating less food, which means less fuel coming in.

For the first few weeks, your body is adjusting to running on noticeably fewer calories. Fatigue is a normal part of that adjustment. The fix is not to fight the appetite suppression, it is to make the food you do eat count. Protein and nutrient density matter far more now than they did before, because you have less room to work with.

You may be underhydrated without realizing it

When you eat less, you drink less. A lot of the water we take in every day comes from food, so when the food goes down, hydration quietly slips too. Add in any nausea or looser stools in the early weeks, and it is easy to end up mildly dehydrated.

Even mild dehydration reads as fatigue, brain fog, and headaches. Before you assume something bigger is going on, look honestly at your water intake. Most of my patients are drinking less than they think.

The nutrient gaps that quietly build up

This is the part most people do not hear about until they are already feeling it. When you eat significantly less for weeks or months, you take in less of everything, including the vitamins and minerals your energy depends on.

B vitamins are central to how your body turns food into usable energy. When intake drops, B vitamin levels often follow, and low B vitamins show up as exactly the kind of persistent tiredness my patients describe. The same goes for other nutrients that support energy and red blood cell health. You are not eating badly on purpose. You are just eating less, and the gaps add up.

This is exactly why I formulated the Jealthy Weight Loss Support Multivitamin the way I did. It is built for people who are eating less and need to protect their nutrient intake while they do. It is not a replacement for food, and it is not a shortcut. It is nutrient support for the reality of eating on a GLP-1.

Your sleep might be off, even if you are in bed the same hours

Rapid changes in eating, digestion, and body composition can disrupt sleep quality even when the number of hours looks fine. If your sleep is lighter or more broken than it used to be, daytime fatigue follows naturally. Pay attention to how rested you feel, not just how long you were in bed.

What I tell my patients to do first

Start with the basics, because most of the time the basics are the answer. Prioritize protein at every meal, since it's the nutrient most likely to fall short and the one your muscles and energy depend on. Stay well hydrated, and if you're eating less on a GLP-1, make sure you're replacing the vitamins, minerals, and nutrients you may not be getting from food.  And protect your sleep the way you would protect any other part of your recovery.

If your fatigue is severe, sudden, or comes with symptoms like a racing heart, shortness of breath, or dizziness, do not troubleshoot it at home. Talk to your prescribing doctor. Sometimes tiredness points to something that needs a real evaluation, and that is worth ruling out.

The bottom line

Feeling tired on a GLP-1 is common, and in most cases it traces back to something practical: eating less, drinking less, and slowly running lower on the nutrients your energy depends on. You do not have to accept exhaustion as the price of your weight loss journey. Cover the basics, support what your body is missing, and give yourself a little grace while you adjust.

This is the exact gap I built Jealthy to help fill. If you want daily nutrient support formulated for people eating less on a GLP-1, the Weight Loss Support Multivitamin and the Wellness Duo are where I would start.
